I have the same problem.
I have an Extreme Control NAC virtual appliance that sends "disconnect request" to the APs but never receives an ACK from the APs.
I have configured the CoA in the WLAN template.
I have captured the traffic on port 3799 and I can only see requests.

To add to this: If the wireless client opens a new browser window, Mist successfully reauthorizes the client. The problem is that it should automatically reauthorize the client after the guest captive portal process completes, but it doesn't do this every time. (The same process works successfully on Aruba.)

I have configured a RADIUS server (PacketFence) to send CoA Re-auth requests. Most of the time the AP seems to not respect the CoA. (It works about once every 5 tries or so.)
Does anyone else have a setup similar to this that works consistently?
So far Mist technical support has not been able to figure out the problem.
