I can confirm that we had this issue in junos 21.4R3-S5.4 before we upgraded to a fixed verion.

Glad to see this annoying bug might be fixed! I'm in the process of updating from 21.4R3-S5.4 to 23.4R2-S3.9 and will see if it's resolved. I never experienced it with EX2300 switches (at least not the 24 port versions), just EX3400 switches.
Also, this PR states it was resolved as far back as 20.4R3 or 21.XR3, but I know I had this issue with 21.4R3-S3.4 as that's what I was running when I posted here in 2023 and currently experience it with 21.4R3.S5.4. Something seems off about that...

Thanks for that PR. However, I don't think it applies to the problem I reported. I was reporting a problem with 'set system ports console log-out-on-disconnect', and only on a subset of ex3400s. And we don't have authentication-order configured under 'system services' hierarchy.
It was an immediate failure to login, but we never got the 'Login Incorrect' message. It just went back to the login prompt after each successful console login attempt. I guess it could somehow be accidentally/tangentially related though.

I've tested and found that this no longer seems to be an issue on 23.4R2-S2.1 version. I searched PRs and could not find any.

BTW, someone else on our team tried other JunOS version and different console cables/USB adapters.
The RMA replacement device is doing the same thing. But I have other ex3400-24p that don't have this problem. Is it a bug with the OS, or a hardware problem with certain hardware revisions of the ex3400-24p? I don't have time to waste figuring it out.
The workaround I used while it was still on the bench is "How to Recover the Root Password on Junos OS with Upgraded FreeBSD", except instead of
`set system root-authentication plain-text-password`
I used
`deactivate system ports console log-out-on-disconnect`

We got a new switch and tried to configure it for production with our standard config. Our standard config contains the command 'set system ports console log-out-on-disconnect'. After committing the config, it would automatically / immediately log out after EVERY successful login with a local account, which is the only option on the bench. This seems like a hardware failure where it sees a console disconnect and logs out even though console is clearly not disconnected, as shown by continued console output (see attachment). This is a repeatable failure.
Has anyone else encountered this failure? It seems like this is possible on any similar hardware.
