Upgrade details are in the release notes for the target version that you want to end with after the upgrade process. Check the upgrade process there along with the known issues for the upgrade process.
As a general rule you can move 3 major versions of Junos, the first number which is also the year of first release, at a time.
You also generally want to be moving from and to an EEOL (extended end of life) version of Junos for that year. These will be either the second number of 3 or 4 depending on the year.
